CHAMPVA Online Filing Steps for VSOs
    1. Go to the VA CHAMPVA online application page:
Apply for CHAMPVA Benefits – VA Form 10-10d
    2. Review eligibility before starting. CHAMPVA may apply to a spouse, dependent child, or survivor of a Veteran who is permanently and totally service-connected, died from a service-connected condition, or was P&T at death. The applicant cannot be eligible for TRICARE. (Veterans Affairs)
    3. Gather sponsor/Veteran information:
    o Veteran’s full name
o Social Security number or VA file number
    o Date of birth
    o Address and phone number
o Date of death, if applicable
    4. Gather applicant information for each spouse/child:
    o Full legal name
    o Social Security number
    o Date of birth
    o Address
    o Phone/email
    o Relationship to Veteran
    5. Gather supporting documents before starting:
o Marriage certificate for spouse/surviving spouse
o Birth certificate for dependent child
    o Adoption papers, if adopted
o Proof of marriage to child’s parent for stepchild
o School certification letter for children age 18–23 in school
o Medicare or other health insurance cards, front and back, if applicable (Veterans Affairs)
    6. Start the online VA Form 10-10d application. VA confirms this form can be completed online instead of mailing the paper form. (Veterans Affairs)
    7. Enter the Veteran sponsor information first.
    8. Add each CHAMPVA applicant. Make sure the relationship is listed correctly, such as spouse, surviving spouse, child, adopted child, or stepchild.
    9. Answer health insurance questions for each applicant. If the applicant has Medicare or other health insurance, VA Form 10-7959c/OHI information is required, along with copies of the insurance card. (Veterans Affairs)
    10. Upload supporting documents directly in the online application. VA states that online applicants can submit documents as part of the online application. (Veterans Affairs)
    11. Review everything carefully before submitting:
    • Names match documents
    • SSNs are correct
    • Dates of birth are correct
    • All dependents are listed
    • Insurance/Medicare information is included
    • Attachments are readable
    12. Submit the application online.
    13. Print or save the confirmation page for the Veteran/family and the office file.
    14. Tell the applicant to watch the mail. If approved, VA will mail the CHAMPVA ID card. (Veterans Affairs)
    15. For questions or status follow-up, CHAMPVA’s customer service number listed on VA Form 10-10d is 1-800-733-8387. (Veterans Affairs)
